Preparing for your Visit

To help ensure your appointment runs smoothly and efficiently, please review the following important information before arriving:

 

  • Check-in: Please check in with reception upon arrival so we can confirm your appointment details and update any necessary information OR try our Self Check-In  Kiosks. 
  • What to bring: Bring your health card, any required paperwork, and all current medications. Although we have a list of medications you’re currently taking, please report any changes or additions made by specialists since your last visit. Also include all non-prescription medications, vitamins, supplements, herbal, and homeopathic remedies. Bringing your pill bottles is very helpful for reviewing how and when medications are taken.
  • Arrival time: Please arrive at least 10 minutes early to allow time for check-in. Being on time is essential and respectful to the doctor, nurses, staff, and other patients.
  • Parking: Parking is available on-site—please allow extra time if needed to find a spot.
  • Location: Our clinic is located on the second floor.
  • Teaching facility: As we are a learning and teaching facility, you may encounter medical students participating in your care.
  • Fees: Some visits or services may have associated fees; please inquire with reception if you have questions.
